Your comments and concerns are important to us
We want to hear from you regarding your satisfaction with our care and services, as well as suggestions for improvement. To help us measure our efforts, we follow up with our patients after their hospital stay, and if we contact you by mail, we hope you will take a moment to respond to our survey questions (provided by NRC Picker). Your comments will help us improve the way we provide care. We also welcome and encourage your comments at any time. All information is used to support our efforts to continually improve the quality of your care and safety. Community Health Needs Assessment (CHNA) and Implementation Strategy
We at Appling Healthcare System are committed to improving the health and wellness of our community through providing exceptional care, as well as offering resources that promote healthy lifestyles.
As a part of this commitment, Community Health Needs Assessments (CHNA) are conducted by collaborating with a distinctive partnership of community stakeholders and Georgia Health Policy Center to better understand and identify the health needs of our community served by Appling Healthcare System. We drive the CHNA with systematic, comprehensive data collection, and analysis.
These documents are made publicly available in compliance with Internal Revenue Service and the Affordable Care Act (ACA) tax law requirements section 501(r). The law requires hospital facilities owned and operated by an organization described in Code section 501(c)(3) to conduct CHNA once every three years.
