Request Medical Records

As a patient, it’s your right to get copies of most medical records. Your medical record includes your health history, allergies, medicines, test results, and how to contact you. So, it’s our job to protect your privacy while giving you timely access when you need it.

The state of Georgia regulates how much we can charge per page to cover the costs of labor, time, and resources needed to prepare your request. Appling Hospital complies with state and federal regulations related to the release of medical record information.

There are five options to request medical records.

If you need assistance, call the Medical Records team at 912-367-9841 ext. 1279. Monday through Friday, from 8 a.m. to 4:30 p.m.

Use the Patient Portal

The patient portal gives you secure online access to your medical record. This option is free for patients.

Online

You can complete the form online and send it through the website or download a copy. A copy of your driver’s license or state issued ID will be requested along with payment, if applicable, prior to medical records being sent.

In-Person, E-Mail, Mail, or Fax

You can also request your records and radiology images in person or by email, mail or fax. You’ll need to complete, sign, and date one of the following: Medical Records Request Form-PDF or Medical Records Release Form-MicrosoftWord.

IN-PERSON: Visit Appling Healthcare’s Medical Records Department, located in the Business Office. The Business Office is located at 34 Walnut Street in Baxley, right next to the main Appling Healthcare hospital building. You will need a driver’s license or state-issued photo ID and the completed release form. If you are having someone else drop off your records request, they will need to show their driver’s license or state-issued photo ID and a copy of the patient’s (your) driver’s license or state-issued photo ID along with the completed release form.

  • MAIL: Mail a copy of your driver’s license or state-issued photo ID and the completed release form to:

Appling Healthcare
Attn: Medical Records
163 E Tollison St.
Baxley, GA 31513

  • FAX: Fax a copy of your driver’s license or state-issued photo ID and the completed release form to 912-366-6030.

After we process the request, we will mail or email you an invoice. Costs vary based on the number of pages released and the records requested. Once the invoice is paid, you will receive a copy of your records. Turnaround times for medical records requests made in person, via fax or email are typically completed within 24 hours of payment in full.

Please note: In providing access or copies of the medical record to the individual, it is our goal to provide records in whole or in part no later than 30 calendar days from receiving the request. If we are unable to provide the records within 30 calendar days, we may extend the time by no more than an additional 30 days. To extend the time, we will provide an explanation in writing to the individual for the reason(s) for the delay and the date the records will be available. This extension must be submitted within the initial 30 days of the original request.
